Mec Header What are the icons in the table header for? This table is where you complete details of your claim. Claim Name: You can overwrite this with your own title or name for the claim. This is something that will enable you to differentiate this claim from other claims you make. Claim Start and End Date: These are the dates between which you were affected. You will be asked to supply independent supporting evidence that relates to these dates. Full Description of Claim: This is a descriptive account which explains why you are submitting your claim. Supporting Document List: List the documentary evidence that you intend to submit in support of your claim. If you do not wish your Director of Student Support to view your claim you can deselect the 'Viewable by Director of Student Support' option. If you wish the Student Life Centre to be made aware of your claim tick the 'Inform Student Life Centre' option. Once you have saved your claim, you will be able to select which assessments were affected. You can save your claim and return to it to make further changes. Once your claim it complete click the 'Submit' button. You will then need to click the 'Print Cover Sheet' button to print the cover sheet and the insert. You should then place the insert and your supporting documents in an envelope, attach the cover sheet to the front of the envelope and submit this to your school office.
